Job summary

La Frontera EMPACT - Suicide Prevention Center is seeking a Part Time SMI Rehabilitation and Discharge Planner to coordinate rehab-related services and provide case management for adults with serious mental illness. The role works with RSA counselors, providers, and treatment teams to implement discharge and transition planning.

Responsibilities include documentation, progress evaluation, and collaboration across inpatient and outpatient settings.

Qualifications

• Bachelor’s degree in Psychology, Social Work, Family Studies, or other behavioral health related field required.
• Master's degree preferred.
• Minimum of 2 years of case management or inpatient experience in behavioral health required.

Responsibilities

• Coordinate rehabilitation-related services for consumers with serious mental illness, and provide case management.
• Collaborate with RSA Vocational Rehabilitation Counselor(s) and providers to implement IGA at direct service level.
• Assess and document needs, develop and adjust service plans, and monitor progress.
• Provide training and support to team on rehabilitation and vocational issues.
• Ensure documentation for each consumer is completed and filed appropriately.
• Assist in discharge planning and coordination with inpatient and outpatient teams.
