Southwest Behavioral & Health Services in the Phoenix area is seeking an SMI Case Manager to provide in-home and community-based support to adults with Serious Mental Illness. You will perform assessments, develop service plans, and coordinate care with a multidisciplinary team to ensure comprehensive treatment and resources are connected.

Responsibilities include crisis intervention, documentation, and transition planning with the care team.

Qualifications

• Bachelor’s degree with one year of full-time behavioral health experience or equivalent
• Fingerprint clearance by Arizona DPS or eligible to receive upon hire
• Experience with multi-cultural and low socio-economic individuals

Responsabilités

• Provide comprehensive in-home/community-based intervention for adults with Serious Mental Illness
• Assess members per policies and clinical protocols
• Develop service plans based on assessment and diagnostic process
• Coordinate member care with the care team and revise plans as needed
• Maintain documentation of contact and services; coordinate with PCP/Health Plan when applicable
• Travel to conduct home visits and attend appointments in the community
