Job summary

JFCS in Phoenix, AZ is seeking an Adult SMI Case Manager to provide ongoing case management for members designated as Seriously Mentally Ill (SMI) receiving behavioral healthcare. The role involves coordinating transportation, services across behavioral and physical health, and navigating patient rights and access to treatment.

The position requires a high school diploma with experience in behavioral health; a Bachelor's in a human services field is preferred, and bilingual Spanish-English is

Qualifications

• Minimum High School Diploma and quality experience in the behavioral-health field; Bachelor's Degree in a human services field preferred.
• Spanish-English bilingual preferred; Pay Differential offered.

Responsibilities

• Performs duties out in the community and provides transportation when necessary.
• Coordinates services for behavioral health and coordination of care with physical health care providers.
• Maintains communication, coordination, and connections with all Clinical Team participants.
• Ensures implementation of “transition to adulthood” referrals.
• Understands and navigates patient rights regarding access to treatment, appeals, and complaint processes.
• Communicates with other staff to ensure coordination and follow-through for clients needing primary care services.
• Performs out-of-office case management services and administrative tasks per agency procedures.
