Job summary

A community service organization in Gilbert, AZ is looking for an Adult SMI Case Manager to provide ongoing case management services for Seriously Mentally Ill clients receiving behavioral healthcare. Responsibilities include coordinating services, ensuring communication among team members, and performing case management in the community. A High School Diploma is required, with a preference for a Bachelor’s Degree. Qualifications

• Minimum High School Diploma with experience in behavioral health.
• Ability to acquire Level 1 Fingerprint Clearance Card.
• AZ Driver's License and reliable transportation required.

Responsibilities

• Coordinate transportation services for members.
• Ensure communication with Clinical Team participants.
• Perform case management services in the community.
