Job summary

Southwest Network, Inc. is seeking a case manager for our adult outpatient clinic.

You will work with a clinical team to engage, educate, and support adults with serious mental illness, coordinating care across behavioral and medical providers to foster independence and meaningful life goals. You will participate in assessing and developing individualized service plans, implementing services, and supporting members toward employment, community involvement, and enhanced self-sufficiency.

Qualifications

• Degree in behavioral health or related field required or preferred
• 1 year related experience with a non-related bachelor's degree accepted
• 1 year related experience with an associate degree accepted
• 3 years related experience with a high school diploma accepted
• Professional certifications may be required or offered through Southwest Network

Responsibilities

• Assess and develop an individualized service plan for members
• Implement identified services to meet recovery goals
• Collaborate with members to build roles outside of illness and pursue employment and community activities
• Provide supportive services and monitor behavioral health services
• Engage with members and families to coordinate resources and care
