Job summary

Homestead Post Acute seeks a Staff Speech-Language Pathologist to evaluate residents, develop and implement individualized treatment plans, and supervise CFY interns. You will work with physicians, families, and a multidisciplinary team to advance communication and swallowing goals, while maintaining compliant documentation.

The role involves educating residents and caregivers, participating in care conferences, and contributing to quality improvement initiatives within the facility.

Qualifications

• Bachelor's degree in Speech-Language Pathology required.
• Masters or Doctorate in Speech-Language Pathology preferred.
• Licensed Speech Therapist in the state and CPR certification as applicable.

Responsibilities

• Evaluate residents to determine speech therapy needs and potential.
• Develop and update individualized treatment plans to improve function.
• Supervise CFY Speech Clinical Fellows and students when applicable.
• Communicate progress, goals, and discharge planning with families and team.
• Complete timely documentation in accordance with regulations and payer requirements.
• Educate residents, families, and caregivers on treatment goals and home programs.
