Northeast Arc, a nonprofit serving people with disabilities in northeastern Massachusetts, seeks a Skills Trainer for the PCM program to help individuals access PCA services and manage care with education and hands-on support.

You will conduct home visits, assess needs, and guide clients through enrolment and employer responsibilities while collaborating with nurses, therapists, and staff to ensure quality service.

Formación

• High School Diploma or equivalent required; BA/BS in Human Service field preferred.
• Prior experience providing service to individuals with disabilities preferred; bilingual a plus.
• Strong interpersonal and customer service skills; detail-oriented.
• Proficient in Microsoft Office applications and entering narrative and other data into a database.
• Valid driver’s license and access to a reliable vehicle.

Responsabilidades

• Assess personal care needs with a nurse and occupational therapist; prepare PCA applications for submission.
• Teach individuals how to hire, train, supervise, and pay PCAs after program approval.
• Provide skills training following the Medicaid curriculum and educate on PCA management.
• Conduct home visits to assess needs and provide in-person support and training.
• Maintain accurate documentation of consumer interactions and progress in the program database.
• Support consumers and surrogates through ongoing case management and respond to changes in care needs.
• Collaborate with the PCM team to ensure coordinated, high-quality service delivery.
• Participate in department meetings and trainings related to the Medicaid PCA Program.
