Job summary

MWVCAA is seeking a dedicated Sheltering Case Manager to support and guide high-need clients through an extended level of housing assistance, aligned with Housing First initiatives. You will conduct eligibility screening, enroll participants, complete required paperwork, and perform data entry while maintaining strict confidentiality.

The role emphasizes collaboration with community partners, rapid housing placement, and ongoing follow-up to remove barriers and secure stable housing.

Qualifications

• Two years of related social service experience or equivalent.
• Advanced degree with two+ years homeless service delivery experience preferred.
• Bilingual capabilities preferred.

Responsibilities

• Conduct housing assessments and assist with program enrollment.
• Provide direct case management and housing placement support.
• Develop participant action plans identifying needs and barriers.
• Maintain strict confidentiality and accurate data entry.
• Coordinate with community partners and assist with annual trainings.
