Job summary

Making Opportunity Count seeks a Shelter Case Manager to provide comprehensive case management for families in shelter. Using motivational interviewing, you will develop individualized rehousing plans to promote self-sufficiency and secure safe, affordable permanent housing.

Cross-trained in the Stabilization Program, you may support operations and ensure continuity of client services, with duties including meetings, document collection, and housing referrals.

Qualifications

• Associate's degree in human services or related field.
• 1 year of case management experience focusing on pre-tenancy and tenancy-sustaining services.
• Empathetic toward diverse socio-economic populations.
• Strong interpersonal and collaboration skills.
• Knowledge of community resources and benefits.

Responsibilities

• Establish monthly rehousing meetings with families.
• Review barriers to housing including credit and criminal records.
• Collect and maintain participant records and documents.
• Create budgets to support housing sustainability.
• Provide education on affordable housing options.
• Document case notes in the client system within 24 hours of meetings.
