Job summary

Making Opportunity Count in Massachusetts seeks a Shelter Case Manager to deliver comprehensive case management for families in shelter. You will use motivational interviewing and strength-based methods to build individualized rehousing plans and measurable steps toward stable, affordable housing.

You may also support Stabilization services as needed, collaborating with staff to ensure continuity of client services and adherence to program protocols.

Qualifications

• Associate's degree in human services, or related field.
• 1 year of experience providing case management services.
• Empathetic to the needs of a diverse population.
• Strong interpersonal and communication skills.
• Knowledge of community resources and benefits.
• Ability to manage multiple cases and prioritize workload.
• Basic math and budgeting skills.
• Team player with program and agency leaders.
• Problem-solving and collaboration abilities.
• Experience with documentation and data systems.

Responsibilities

• Establish monthly rehousing meetings with families.
• Provide notices of meetings per program rules.
• Review barriers to housing and assist with finances and records.
• Collect vital records and documents and maintain participant files.
• Review tenancy preferences and needs with clients.
• Create budgets to ensure housing sustainability.
• Document case work and meetings within 24 hours in ETO.
• Educate about affordable housing options and refer services.
• Update weekly reports and housing placement reports.
• Conduct home visits to assess stability and connect with resources.
• Provide community-based case management when assigned.
