Qualifications

• Bachelor’s degree preferred in human services or related field.
• Two years’ experience in human services or related field required.
• Demonstrated ability to work with participants experiencing homelessness, poverty, mental health, and substance use issues.

Responsibilities

• Provide weekly housing-focused case management to a caseload of 18-24 individuals.
• Develop housing plans with participants to exit shelter and stabilize housing.
• Assist with intake, documents, and CAN guideline adherence.
• Connect participants to benefits and supportive services.
• Maintain accurate case management logs and HMIS documentation.
