Job summary

Making Opportunity Count in MA is seeking a Shelter Case Manager to provide comprehensive case management for families in shelter. You will develop rehousing plans, coordinate meetings, and document progress in a timely manner to promote stable housing. Cross-training in Stabilization may occur.

The role emphasizes collaboration with program staff to address barriers, gather documents, and connect families with resources. A strong emphasis on empathy and community partnerships is required.

Qualifications

• Associate's degree in human services or related field.
• 1 year of experience providing case management services.
• Empathetic to diverse socio-economic populations.
• Strong interpersonal skills and collaboration ability.
• Knowledge of community resources and benefits.
• Ability to manage multiple cases and priorities.
• Good communication and collaboration with providers.
• Strong internet, database, and computer skills.

Responsibilities

• Establish monthly rehousing meetings with families.
• Provide accurate written meeting notices per rules.
• Review barriers to housing and document needed records.
• Collect records and maintain participant files in a timely manner.
• Create a budget to ensure housing sustainability.
• Document case management efforts in ETO within 24 hours.
• Update housing reports and address program compliance.
• Conduct home visits to assess housing stability.
