Job summary

Housing Works, Inc. seeks a Housing Case Manager to deliver comprehensive case management and housing-focused services to single adults in DHS shelters. You will assess, plan, assist with benefits, and provide housing placement support for a caseload of up to thirty clients.

You will collaborate with internal programs, maintain records, and support clients toward stability and permanent housing while upholding confidentiality and harm-reduction principles.

Qualifications

• Bachelor’s degree preferred in social work, psychology, counseling, or related field, or 3 years relevant experience.
• Experience with homeless adults, including those with mental illness, substance use, or immigration backgrounds.
• Flexible hours including occasional overtime.
• Background checks required.
• Strong crises management and de-escalation skills.
• Familiarity with DHS systems (CARES, HRA OneSite) preferred.
• Proficiency in Microsoft Office Suite.

Responsibilities

• Provide case management for up to 30 single adults in DHS shelters.
• Develop treatment plans and assist with benefits and housing placement.
• Maintain records per funder and regulatory requirements.
• Provide referrals for medical, mental health, substance use, and financial needs.
• Collaborate with internal programs and support client transitions to housing.
• Develop strength-based goals and monitor participation.
• Complete paperwork and reports by deadlines.
