Job summary

The Salvation Army North and Central Illinois Division is seeking a Case Manager for the Safe Harbor Shelter in Bloomington, IL. You will provide comprehensive case management, assess client needs, develop plans, and liaise with community resources to support individuals experiencing homelessness.

This full-time, non-exempt role emphasizes accurate documentation, confidentiality, and collaboration with diverse teams to advance grant outcomes and program requirements.

Qualifications

• Bachelor's degree in social work, sociology, criminal justice, or related field; or five years relevant experience.
• Minimum of 3 years experience working with homelessness or vulnerable populations with strong documentation.
• Proficient in written and verbal communication; able to work with diverse populations.

Responsibilities

• Review intake material for accuracy and assess client needs.
• Develop and monitor case plans and ensure compliance.
• Coordinate referrals to community resources and arrange transportation.
• Maintain client records with confidentiality andHMIS data entry.
• Attend monthly meetings and participate in on-call rotation.
