Job summary

The Salvation Army Midland Division Alton Shelter in Alton, IL is seeking a Shelter Case Manager to provide case management services to shelter clients and ensure compliance with active grant requirements. This full-time role (40 hours/week) involves intake, planning, data entry in SIMS, referrals, and coordinating services with community partners.

Ideal candidates hold a bachelor’s degree in social work or related field with at least one year of relevant experience, strong interpersonal skills,

Qualifications

• Requires a bachelor’s degree in social work, psychology or related field and at least one year of relevant experience.
• Excellent interpersonal and communication skills; ability to work with diverse populations.
• Ability to type case notes and update Excel spreadsheets; HMIS training within 3 months.
• Driver license and pass TSA MVR check is a plus.

Responsibilities

• Conduct initial intake assessment to determine eligibility and client needs.
• Provide case management services using Pathway of Hope and grant requirements.
• Enter client data into SIMS and maintain case records per agency standards.
• Offer information/referral to remove barriers and assist with housing options.
• Develop discharge plans and coordinate services with partner organizations.
