Job summary

The Salvation Army North and Central Illinois Division in Bloomington, IL is seeking a dedicated Residential Services Case Manager to provide comprehensive case management for clients experiencing homelessness and others in need. You will review intake materials, assess needs, develop and monitor case plans, and coordinate referrals.

You will maintain confidential records, document progress in HMIS Service Point, participate in monthly CIOC meetings, and support Safe Harbor programs while

Qualifications

• Bachelor of Science in social work, sociology, criminal justice or related field; or five years’ case management experience.
• Minimum of 3 years experience with homelessness or vulnerable populations.
• Proficiency in case management, documentation, timely reporting, recordkeeping and program compliance.

Responsibilities

• Provide case management, assess needs, develop and monitor case plans.
• Liaise with community resources and provide referrals to appropriate agencies.
• Ensure clients have access to case management via appointments and walk-ins.
• Document information and progress in HMIS Service Point; submit required statistics.
