Job summary

The Salvation Army North and Central Illinois Division is seeking a Case Manager for Safe Harbor Shelter in Bloomington. You will review intake materials, assess needs, develop and monitor case plans, and coordinate referrals to community resources.

Qualifications

• Bachelor's degree in social work or related field, or five years of relevant experience.
• Minimum 3 years worked with homelessness or vulnerable populations.
• Proven case management, documentation, and compliance skills.

Responsibilities

• Provide case management, intake review, needs assessment, and case plan development.
• Liaise with community resources, arrange referrals and transportation.
• Document client information in HMIS Service Point and maintain records.
