Qualifications

• Requires a bachelor’s degree in social work, psychology or related field and at least one year of human services experience.
• Excellent interpersonal and communication skills; able to maintain confidentiality and professional boundaries.

Responsibilities

• Conduct initial intake assessment to determine client needs and eligibility.
• Provide case management using Pathway of Hope within grant requirements.
• Identify housing options and follow-up services for clients; develop safe discharge plans.
• Enter data into SIMS and maintain client files and progress notes.
• Provide information and referrals to address barriers such as health, education and housing.
