Job summary

The Salvation Army Midland Division Alton Shelter in Alton, IL is seeking a Shelter Case Manager to provide case management services to shelter residents and ensure compliance with active funding requirements.

This full-time role (40 hours/week) uses the Pathway of Hope approach, maintains SIMS records, coordinates housing options, and supports clients toward stable, permanent housing while collaborating with community partners.

Qualifications

• Bachelor’s degree in social work, psychology or related field with at least one year in human services.
• Strong interpersonal and communication skills; able to work independently with minimal supervision.
• Must obtain CPR/First Aid and Bloodborne Pathogens certifications; HMIS training within 3 months.
• Valid driver license is preferred; ability to network with community organizations.

Responsibilities

• Conduct initial intake assessments to determine client needs and eligibility.
• Provide case management using Pathway of Hope and grant requirements.
• Enter data into SIMS; maintain confidential client records and tools.
• Identify housing options and follow-up services for clients.
• Develop discharge plans; coordinate with community resources and agencies.
• Meet with supervisor regularly to coordinate client care and documentation.
