Job summary

Homes for Independent Living, a MyPath company, seeks a Service Coordinator to ensure delivery of client services across programs, supervise staff, and manage household operations in Green Bay, WI. The role requires coordinating care, emergencies, budgeting, and staff development while traveling between locations as needed.

The ideal candidate has experience in long-term care, supervisory roles, strong communication, and organizational skills, with a valid Wisconsin license and reliable

Qualifications

• High School Diploma or equivalent required; Bachelor’s preferred.
• Valid Wisconsin driver’s license with 3 years of acceptable driving record.
• Personal vehicle with insurance.
• 3 years experience in long term care setting and 2 years supervisory experience.

Responsibilities

• Develop and manage individual service plans (ISPs) for assigned clients.
• Coordinate delivery of services across multiple programs according to ISP.
• Respond to program emergencies including after-hours situations.
• Oversee household operations: maintenance reports, menus, shopping, housekeeping, and activity planning.
• Perform HR and payroll functions for direct reports, including interviews, onboarding, training, performance monitoring, and coaching.
