Job summary

MyPath is seeking a Service Coordinator in Green Bay, WI to oversee delivery of client services, supervise staff, and coordinate across multiple residential programs. The SC will develop ISP plans, respond to emergencies, train staff on treatments, and manage household operations and client funds.

The role requires strong communication, organization, and a willingness to travel to work locations and trainings.

Qualifications

• High School Diploma or equivalent, Bachelor’s preferred.
• Valid Wisconsin driver’s license with 3 years driving record.
• Experience in long term care and supervisory roles.

Responsibilities

• Develop and manage individual service plans for clients.
• Ensure services align with ISP across programs.
• Respond to emergencies outside normal hours.
• Train staff on treatments and logging; manage household operations.
• Oversee HR and payroll for direct reports; coaching and action plans.
• Manage client funds and valuables responsibly.
