Job summary

Homes for Independent Living, a MyPath company, is seeking a Service Coordinator in West Bend, WI to oversee client services and household operations. You will develop and manage individualized service plans, supervise staff, and act as primary liaison between guardians, clients, and the organization.

The position requires strong communication, organizational skills, travel as needed, and a background in long-term care.

Qualifications

• High School Diploma or equivalent; Bachelor's preferred.
• Valid Wisconsin driver's license with clean driving record.
• 3+ years in long-term care setting with supervisory experience.

Responsibilities

• Develop and manage individual service plans (ISP) for assigned clients.
• Ensure delivery of services outlined in ISP across programs.
• Respond to program emergencies, including after hours.
• Oversee household operations: menus, shopping, housekeeping, vehicles, activities.
• Perform HR and payroll functions for direct reports: interviews, training, coaching.
• Manage client funds and valuables responsibly.
