Job summary

MyPath seeks a Service Coordinator to oversee client services, develop ISP plans, supervise staff, and coordinate care across programs. The role acts as the primary liaison between guardians, clients, and HIL and involves ensuring service delivery per ISP and managing household operations.

The position requires strong communication and organizational skills, plus the ability to travel to different locations and respond to emergencies as needed.

Qualifications

• Must have excellent communication and organizational skills.
• Experience in long term care setting and supervisory roles preferred.
• Ability to travel to different work locations, training sessions, and meetings.

Responsibilities

• Develop and manage ISP for assigned clients.
• Ensure ISP-delivered services across programs.
• Respond to emergencies including outside normal hours.
• Manage medical treatment supplies and train direct care staff.
• Serve as primary contact for hospital staff during emergencies.
