Job summary

CommonBond Communities in Newport, MN seeks a Service Coordinator II to provide independent service coordination for residents and promote community building within permanent supportive housing settings. You will conduct strengths-based assessments, develop resident-centered plans, respond to crises, and coordinate with property partners to support tenancy and engagement.

Qualifications

• Minimum 2 years experience working with diverse communities.
• Excellent interpersonal, verbal, and written communication skills.
• Knowledge of community resources and service networks.
• Ability to travel between sites.
• Be CPR and First Aid certified or able to become certified within six months.
• Experience using MS Office 365 and other software programs.

Responsibilities

• Provide independent service coordination for residents, including those in permanent supportive housing when assigned.
• Conduct strengths-based assessments and develop resident-centered service plans.
• Respond to resident crises independently with needed consultations.
• Manage a defined caseload of permanent supportive housing households.
• Collaborate with 3rd-party property management to address lease violations and coach residents back to good standing.
• Coordinate multi-party case conferences for complex tenancy issues.
• Maintain timely, complete records in ETO and ensure program compliance.
• Engage residents in community programming and partnerships.
