Job summary

CommonBond Communities in Minnesota seeks a Service Coordinator II to provide independent service coordination for residents, including those in permanent supportive housing, and to promote community building and engagement.

The role emphasizes motivational interviewing, person-centered planning, compliance, budget stewardship, and collaboration with property management and community partners. This is a full-time position requiring strong communication and site-to-site travel.

Qualifications

• Minimum 2 years’ experience working within diverse communities.
• Bachelor’s degree in social work or related field OR 2–4 years related experience.
• Excellent interpersonal and written communication skills.
• Ability to travel between sites.

Responsibilities

• Conduct strengths-based assessments and develop resident-centered service plans using motivational interviewing.
• Respond to resident crises independently, with consultation as needed; coordinate warm handoffs to clinical or emergency services.
• Manage a defined caseload of permanent supportive housing households with proactive outreach and engagement.
• Collaborate with 3rd-party property management to address lease violations and coach residents back to good standing.
• Maintain timely, accurate resident records and contribute to data quality.
