Job summary

CommonBond Communities is seeking a Service Coordinator II to provide independent service coordination for residents, including those in permanent supportive housing, and to promote community building and engagement.

You will conduct strengths-based assessments, develop resident-centered plans, and coordinate with property management partners while maintaining accurate records. A collaborative, client-focused approach and CPR/First Aid certification are preferred.

Qualifications

• Minimum 2 years demonstrated experience working with diverse communities.
• Knowledge of community resources and service networks.
• Excellent interpersonal, verbal, and written communication skills.
• Ability to work in a collaborative team environment with internal and external partners.
• CPR and First Aid certification or ability to obtain within six months.

Responsibilities

• Provide independent service coordination for residents.
• Promote community building and engagement with residents.
• Conduct strengths-based assessments and planning.
• Coordinate with property managers and partners on tenancy issues.
• Maintain timely, accurate resident records in ETO.
