Job summary

CommonBond Communities seeks a Part Time Service Coordinator II at the Dublin Crossing site in Mankato, MN. The role provides independent service coordination for residents, including those in permanent supportive housing, with emphasis on motivation interviewing and person-centered planning.

You will conduct assessments, manage a defined caseload, and collaborate with housing partners to resolve tenancy issues while empowering residents to engage in community programming.

Qualifications

• Bachelor's degree in social work or related field (or equivalent experience).
• Minimum 2 years of experience serving diverse communities and coordinating resident services.
• Licensed mental health professional or social worker preferred.

Responsibilities

• Conduct strengths-based assessments and develop resident-centered service plans.
• Respond to crises independently with escalation as needed; arrange warm handoffs to clinical services.
• Manage a defined caseload in permanent supportive housing with proactive outreach.
• Coordinate with third-party property management to address issues and coach residents back to good standing.
• Engage residents through monthly communications and inclusive program design.
• Maintain timely and accurate resident records in accordance with program standards.
• Collaborate with team members and partners to ensure coordinated tenancy support.
