Job summary

CommonBond Communities seeks a Service Coordinator II to provide independent service coordination for residents, including those in permanent supportive housing. The role focuses on motivational interviewing, person-centered planning, budget stewardship, and building community engagement.

You will collaborate with property management partners and contribute to team objectives while maintaining compliant, client-centered records across sites.

Qualifications

• Minimum 2 years demonstrated experience working within diverse communities
• Knowledge of community resources and service networks
• Excellent interpersonal, verbal, and written communication skills
• Ability to work well in a collaborative team environment with internal and external partners
• Be certified in CPR and First Aid or able to become certified within six months of starting position

Responsibilities

• Conduct strengths-based assessments and develop resident-centered service plans using motivational interviewing
• Respond to resident crises independently with escalation as needed
• Manage a defined caseload of permanent supportive housing households with outreach and engagement
• Collaborate with third-party property management to address lease violations and coach residents back to good standing
• Inform residents of information and programming opportunities via monthly newsletters and gather feedback for program improvements
• Operate with moderate autonomy, prioritize caseload, and adhere to program standards
• Maintain timely, accurate records in ETO and ensure program compliance
