Job summary

CommonBond Communities is seeking a Service Coordinator II to provide independent service coordination for residents, including those in permanent supportive housing. You will focus on motivational interviewing, person-centered planning, and ensuring program compliance while promoting community engagement.

Responsibilities include managing a defined caseload, coordinating with property management partners, and empowering residents through information and activities.

Qualifications

• Minimum 2 years demonstrated experience working within diverse communities.
• Knowledge of community resources and service networks.
• Excellent interpersonal, verbal, and written communication skills.
• Ability to network and develop community partnerships.
• Experience using MS Office 365.
• Be certified in CPR and First Aid or can become certified within six months of starting position.

Responsibilities

• Conduct strengths-based assessments and develop resident-centered service plans using motivational interviewing.
• Respond to resident crises independently, with consultation as needed; coordinate warm handoffs to clinical or emergency services.
• Manage a defined caseload of permanent supportive housing households with proactive outreach and engagement.
• Collaborate with 3rd-party property management to address lease violations and coach residents back to good standing.
• Inform residents of site information and programming via a monthly newsletter and communications.
• Listen to residents and gather ideas on programming and services they want to see.
• Involve residents in activities and programs within their housing community.
• Empower residents to lead on issues they care about within their community.
• Maintain timely, complete records in ETO and ensure program compliance.
• Travel between sites as required and work with internal/external partners.
