Job summary

CommonBond Communities is seeking a Service Coordinator II at Red Rock Manor to provide independent service coordination for residents and support community building. The role emphasizes motivational interviewing, person-centered planning, and compliance with Housing Support programs.

You will coordinate with third-party partners, manage a defined caseload, and help residents access housing services and resources.

Qualifications

• Bachelor's degree or 2–4 years related work experience.

Responsibilities

• Provide independent service coordination for residents, including those in permanent supportive housing when assigned.
• Promote community building and engagement through person-centered planning and motivational interviewing.
• Respond to resident crises independently with available consultation.
• Coordinate warm handoffs to clinical or emergency services as needed.
• Manage a defined caseload and collaborate with property management partners to address tenancy issues.
• Coordinate multi-party case conferences for complex tenancy issues.
• Inform residents of program information through newsletters and routine communications.
• Engage residents in activities, programs, and opportunities within their housing community.
• Maintain timely, accurate resident records in ETO and adhere to compliance standards.
• Collaborate with team members, provide needed information promptly, and support a respectful, growth-oriented workplace.
