Job summary

CommonBond Communities is seeking a Site Service Coordinator III to lead resident-centered services and housing stability efforts at a City, State site. You will coordinate assessments, plans, and outreach, while promoting community engagement and collaboration with housing partners.

As a senior member of the team, you will mentor colleagues, manage complex cases, and ensure documentation accuracy within HMIS/ETO systems. A strong commitment to equity and community impact is essential.

Qualifications

• Bachelor's degree in social work or related field OR at least four years of work experience in a related field.
• Licensed mental health professional or social worker.
• Experience with individuals and families transitioning out of homelessness and coordinated entry systems.
• Knowledge of affordable housing programs.
• CPR and First Aid certification or willingness to obtain within six months.

Responsibilities

• Lead comprehensive assessments and service plans for complex resident needs.
• Coordinate crisis intervention and safety planning.
• Link residents with community-based services and resources to address barriers.
• Provide case management for permanent supportive housing populations.
• Mentor and coach team members; conduct QA checks on documentation.
