Job summary

The Lead Registered Nurse Case Manager (RNCM) at PACE Southeast Michigan leads the nursing team in delivering comprehensive home-based care to frail elders with complex needs. The RNCM oversees assessments, plans care, supervises staff, educates participants and families, and coordinates with the multidisciplinary team to optimize outcomes.

Qualifications

Must hold a current Michigan RN license.
BSN preferred.
Minimum 1 year of experience with frail/elderly population.
Ability to lead licensed and non-professional staff.
Must have valid driver’s license and reliable transportation.

Responsibilities

Lead and supervise licensed and non-professional nursing staff in home-based care.
Assess participants’ needs and coordinate plan of care with families and MDT.
Administer medications as ordered and monitor responses.
Maintain accurate records per CSI policies and CMS competencies.
Participate in quality assurance projects and care plan reviews.
