PACE Southeast Michigan is seeking a Registered Nurse Case Manager to coordinate care for frail elders in the PACE SEMI program. You will assess needs, plan and supervise nursing care, and lead licensed and non-professional staff within the home setting.

Responsibilities include teaching families, coordinating with the interdisciplinary team, and ensuring adherence to care plans and safety protocols while promoting autonomy for participants.

Qualifications

• Must be a Registered Nurse with current Michigan licensure.
• BSN preferred for this role.
• Minimum 1 year of experience with a frail or elderly population.

Responsabilités

• Assess participants’ needs and plan nursing care during Intake and Re-Evaluation Assessments.
• Collaborate with participant, family and Multidisciplinary Team to develop the plan of care.
• Educate, supervise and counsel participant or caregiver on nursing care needs and medications.
