PACE Southeast Michigan seeks a Lead Registered Nurse Case Manager to coordinate nursing care for frail elders in the PACE SEMI program. You will assess needs, plan care, and lead licensed and non-professional staff in delivering comprehensive home-based care.

You will educate participants and families, monitor treatments, and collaborate with an interdisciplinary team to optimize functional outcomes and autonomy.

Qualifications

• Must be a Registered Nurse with current Michigan licensure.
• BSN preferred.
• One year of experience with frail or elderly population.

Responsabilités

• Assess participants’ needs and plan nursing care.
• Collaborate with participant, family and multidisciplinary team.
• Lead and supervise nursing staff in home care delivery.
• Administer medications and treatments as ordered.
• Provide education to participants, caregivers, and family.
