Job summary

PACE Southeast Michigan (PACE SEMI) is seeking a Registered Nurse Case Manager to coordinate care for frail elders in the community and home setting. You will assess needs, plan care, and collaborate with a multidisciplinary team to improve outcomes and maintain independence.

Responsibilities include supervising staff, teaching participants and families, administering medications per orders, and supporting quality improvement initiatives in alignment with CMS and PACE standards.

Qualifications

• Must be a Registered Nurse with current Michigan licensure.
• Minimum 1 year of experience with a frail or elderly population.
• BSN preferred and annual mandatory in-service training.
• Must have valid Michigan driver’s license and reliable transportation.

Responsibilities

• Assess participants’ needs and plan appropriate nursing care.
• Collaborate with the participant, family, and multidisciplinary team.
• Lead and supervise staff in delivery of nursing care in the home.
• Educate participants and caregivers on nursing care and medications.
