Job summary

PACE Southeast Michigan is seeking a Registered Nurse Case Manager to coordinate care for frail elders in the PACE SEMI program. You will assess needs, plan and implement nursing care, and collaborate with a multidisciplinary team to optimize participant outcomes and autonomy.

You will educate participants and families, administer medications per orders, and supervise home health staff. A Michigan RN license and valid driver’s license are required, with BSN preferred.

Qualifications

• Must be a Registered Nurse with current Michigan licensure.
• BSN preferred.
• Minimum 1 year experience with frail/elderly population.
• Ability to supervise and guide licensed and non-professional staff.
• Must have reliable transportation and valid driver's license.

Responsibilities

• Assess participants’ needs and plan appropriate nursing care.
• Collaborate with the multidisciplinary team to develop the care plan.
• Educate participants and families using adult learning principles.
• Administer medications and monitor responses as ordered by physician/NP.
• Lead and supervise home health staff and coordinate in-home care.
