Job summary

PACE Southeast Michigan in Michigan is seeking a Lead Registered Nurse Case Manager (RNCM) to lead and coordinate care for frail elders in the PACE SEMI program. You will supervise licensed and non-professional staff and guide the nursing process from assessment through evaluation.

Qualifications

• Must be a Registered Nurse with current Michigan licensure.
• One year of experience with frail or elderly population.
• Driver''s license and reliable transportation.
• BSN preferred but not required.

Responsibilities

• Assess participants'' needs and plan appropriate nursing care.
• Collaborate with participant, family, and multidisciplinary team to develop care plan.
• Maximize participant functional capacity and autonomy.
• Teach, supervise, and counsel participant or caregiver on nursing care needs.
• Administer medications and treatments as ordered; monitor response.
• Lead and monitor staff in delivery of care; document outcomes.
• Coordinate IDT and ensure quality assurance and data collection.
• Participate in on-call rotation and after-hours support.
