Job summary

PACE Southeast Michigan in Dearborn, MI is seeking a Registered Nurse Case Manager to coordinate care for frail elders in the PACE program. The RNCM conducts assessments, plans of care, and leads a multidisciplinary team to improve functional capacity and autonomy.

Responsibilities include teaching patients and families, supervising home health aides, monitoring medications, and ensuring documentation meets CSI standards.

Qualifications

• Must be a Registered Nurse with Michigan licensure.
• One year of experience with a frail or elderly population.
• BSN preferred and a valid driver's license.

Responsibilities

• Assess participant needs and plan appropriate nursing care.
• Collaborate with family and multidisciplinary team on care plans.
• Lead licensed and non-professional staff in delivering home-based care.
• Administer medications as ordered and monitor responses.
• Maintain accurate records per CSI policies and evaluate outcomes.
