Job summary

PACE Southeast Michigan is seeking a Registered Nurse Case Manager to coordinate care for frail elders in the PACE SEMI program. The RNCM leads licensed and non-professional staff, conducts assessments, and plans comprehensive nursing care including prevention and treatment strategies.

Responsibilities include education for participants and families, medication management, and collaboration with the multidisciplinary team to optimize functional capacity and autonomy in daily activities.

Qualifications

• Must be a Registered Nurse with current Michigan licensure.
• BSN preferred.
• One year of experience with a frail or elderly population.
• Must have a valid Michigan driver’s license and acceptable driving record.
• Ability to lead and direct other licensed and non-professional staff.
• Knowledge of CMS competencies and standards.

Responsibilities

• Assess participants’ needs and plan appropriate nursing care.
• Collaborate with participants, families, and multidisciplinary team to develop the plan of care.
• Educate, supervise, and counsel participants or caregivers on nursing care needs.
• Administer medications and treatments as ordered by physician/NP; monitor response.
• Notify medical personnel of status changes and ensure safe home administration of meds.
• Maintain accurate records; document outcomes and care plan revisions.
• Contribute to QA/data collection and participate in professional meetings.
