Job summary

PACE Southeast Michigan is seeking a Registered Nurse Case Manager to coordinate and deliver comprehensive nursing care for frail elders in the PACE SEMI program. The RNCM will assess needs, plan and implement care, supervise home health staff, and engage families to promote autonomy and optimal outcomes.

The role requires Michigan RN licensure, a BSN preferred, and at least one year of experience with frail elderly populations.

Qualifications

• Must be a Registered Nurse with current Michigan licensure.
• BSN preferred.
• One year of experience with a frail or elderly population.
• Must have a valid Michigan driver’s license.

Responsibilities

• Assess participants’ needs and plan care at Initial Intake and Re-Evaluation Assessments.
• Collaborate with participant, family and multidisciplinary Team to develop care plan.
• Lead and monitor licensed and non-professional staff in delivery of nursing care in the home.
• Provide education to participants or caregivers using adult learning principles.
