Qualifications

Responsibilities

• Provide highly complex nursing case management for individuals.
• Coordinate care with IDT, LARs, and direct care staff.
• Lead nursing team, supervise RNCMs, and mentor staff.
• Maintain comprehensive nursing documentation and care plans.
• Support admissions, transfers, and specialized clinics.
