Job summary

The Texas Health and Human Services Commission (HHSC) seeks a Registered Nurse Case Manager at the Abilene State Supported Living Center. This role involves advanced nursing work, case management across complex health needs, and leadership in nursing staff coordination.

Qualifications

• Licensed to practice as a registered nurse in Texas or a reciprocal state.
• Bachelor's degree is preferred.
• At least one year of RN experience required; two years preferred.
• Experience with individuals with intellectual and/or developmental disabilities preferred.

Responsibilities

• Provide highly complex RN case management for individuals with specialized medical/behavioral needs.
• Lead nursing team, provide training, coaching, and technical assistance.
• Coordinate care with IDT, LARs, direct care staff, and health care providers.
• Document thoroughly and maintain required nursing records and care plans.
