Job summary

Texas Health and Human Services Commission (HHSC) is seeking a Nurse III Registered Nurse Case Manager in Lubbock. This role provides highly complex nursing case management for residents with medical and behavioral needs at the Lubbock State Supported Living Center, coordinating with the IDT and LARs to ensure person-centered care.

The RNCM position requires Texas RN licensure, strong assessment and leadership skills, and experience coordinating multi-disciplinary care.

Qualifications

• Texas RN license required.
• Experience with individuals who have IDD or complex medical needs preferred.
• Ability to coordinate care with the IDT and LARs.

Responsibilities

• Provide highly complex nursing case management for individuals on caseload.
• Develop and evaluate care plans and nursing interventions.
• Lead, train, and mentor nursing staff and direct care teams.
• Coordinate admissions, transfers and medical appointments to ensure continuity of care.
