Job summary

Texas Health and Human Services Commission in Brenham is seeking a Registered Nurse Case Manager (RNCM) to provide advanced clinical nursing care for individuals with complex needs and to coordinate services across the care team.

You will lead nursing staff, conduct comprehensive assessments, develop care plans, and collaborate with the IDT, guardians, and LARs to ensure high-quality, person-centered care in a state-supported living setting.

Qualifications

• Licensed to practice as a registered nurse in Texas.
• Bachelor's Degree in Nursing preferred; TX RN License required.
• Experience in a health agency or hospital setting preferred.

Responsibilities

• Perform highly complex nursing case management for individuals with specialized medical and behavioral health needs.
• Coordinate care with the IDT, LARs, direct care staff, and physicians to support continuity of care.
• Develop, evaluate, and monitor nursing interventions and care plans; ensure compliance with policies and quality standards.
• Provide leadership, training, and technical assistance to nursing and direct care staff.
• Assist with admissions, transfers, medical appointments, and specialized clinics.
