Job summary

Centene Corporation in Abilene, TX is seeking a care manager to assess and coordinate services for high-acuity members. The ideal candidate holds RN or NP licensure, a nursing degree, and 4–6 years of related experience; RUG certification within 90 days of hire is required.

Responsibilities

• Evaluates the service needs of the most complex or high risk/high acuity members and recommends a plan for the best outcome
• Develops and continuously assesses long-term care plans / service plans and collaborates with care management team to identify providers, specialists, and/or community resources needed to address member’s needs
• Coordinates and manages between member/family and care provider team to ensure person-centered care
• Monitors care plans and member status, change in condition, and progress towards goals
• Reviews member data to identify trends and improve operating performance and quality care
• Collaborates with healthcare providers to facilitate member services and revised plans
• Educates members and families on disease processes, care gaps, and benefit options
• Acts as liaison between member/family, physician, and facilities/agencies
• Provides coordination of service authorization for various services
• Performs home visits as needed
• Engages with leadership to improve quality and service delivery
