Job summary

The Texas Health and Human Services Commission is seeking a Registered Nurse Case Manager for the Brenham State Supported Living Center. You will deliver complex nursing case management, assess health needs, coordinate care, and support residents with disabilities in a state facility.

Qualifications

• Licensed to practice as a registered nurse in Texas.

Responsibilities

• Performs highly complex nursing case management for individuals with specialized health needs.
• Coordinates care with the IDT, LARs, direct care staff, nursing staff, and providers.
• Provides leadership, training, and hands-on nursing care as needed.
