Centene is seeking a Senior Care Manager RN in Texas to assess, plan, and implement complex care management for members with chronic conditions. The role develops personalized care plans, educates members and caregivers, and collaborates with providers to coordinate services and resources at high quality while controlling costs.

Responsibilities include monitoring plans, addressing barriers, and ensuring regulatory compliance.

Qualifikationen

• RN-licensed nurse with 4–6 years in clinical nursing or care coordination.
• Experience with state programs (Medicaid/STAR/CHIP) a plus.
• Strong documentation and communication skills.

Aufgaben

• Develops and continuously assesses care plans in collaboration with providers to address unmet needs.
• Coordinates between members, families, and care teams to ensure person-centered care.
• Monitors progress towards goals and revises plans as needed to meet member needs.
• Identifies problems/barriers for care management and initiates interventions for escalated cases.
• Reviews member data to identify trends and improve quality and compliance with regulations.
• Performs outreach (phone, telehealth, home visits) to assess needs and coordinate resources.
• Collaborates with healthcare providers to adjust care plans as needed.
• Maintains accurate member information and ensures regulatory compliance.
