About This Role

The Senior Care Manager RN role involves assessing, planning, and implementing complex care management activities to enable quality, cost-effective healthcare outcomes for members with physical health conditions and chronic disease management. This position is ideal for experienced nurses who are passionate about providing personalized care and support to members and their families.

• Pay: $75,300 - $135,400/year
• Location: Remote, TX
• Schedule: Full Time, Day shift

What You'll Do

• Develop and continuously assess ongoing care plans/service plans.
• Collaborate with providers to identify necessary resources for members.
• Monitor care plans and member status, revising as necessary.
• Identify problems/barriers for care management and interventions.
• Review member data to identify trends and improve care quality.
• Develop appropriate care plans/service plans based on referrals and assessments.
• Perform telephonic, digital, and home outreach to assess member needs.
• Collaborate with healthcare providers to facilitate member services.
• Document and maintain member information to ensure compliance.
• Provide education to members and their families on healthcare options.

Education

• Degree from an Accredited School of Nursing or Bachelor's degree in Nursing required.

Experience

• 4 – 6 years of related experience required.
• 7+ years of clinical nursing experience preferred.

Licenses & Certifications

• RN - Registered Nurse - State Licensure and/or Compact State Licensure required.
