Qualifications

• Requires graduate from an accredited School of Nursing and 4–6 years of related clinical nursing experience.
• Bachelor's degree in nursing (BSN) is preferred.
• RN licensure required or equivalent state licensure; RUG certification within 90 days may be required.

Responsibilities

• Evaluates the service needs of the most complex or high risk/high acuity members and recommends a plan for the best outcome.
• Develops and continuously assesses long-term care plans and collaborates with the care management team to identify providers and resources.
• Coordinates between members, families/caregivers and care providers to ensure patient-centered care.
• Monitors and revises care plans and member status to meet goals and needs.
• Educates members and families on disease processes, care options, and available services and benefits.
• Acts as liaison between member/family, physicians, and facilities.
