Job summary

Centene Corp. is seeking a Senior Care Manager RN for STAR/ CHIP to assess, plan, and implement complex care management for members in Texas.

The role focuses on personalized care plans, education for members and caregivers, and coordination with providers and resources to achieve high-quality, cost-effective outcomes. Ideal candidates have RN licensure with 4–6 years of clinical nursing and care coordination, with BSN preferred.

Qualifications

• Graduate of an Accredited School of Nursing with 4–6 years of clinical nursing, direct RN case management, and/or care coordination experience.
• BSN is preferred.

Responsibilities

• Develops and updates individualized care plans and coordinates with providers to secure needed resources.
• Monitors member status and progress toward goals; revises plans as needed with members and caregivers.
• Educates members and families on disease processes, care options, and benefits; documents and maintains records for compliance.
• Precepts new hires, supports onboarding, and collaborates with leadership to improve care delivery and outcomes.
